چکیده

Quantitative determination of catecholamines and their O-methyl metabolites plays an important role in the diagnosis of pheochromocytoma (PHEO) – adrenal medulla tumour. This kind of tumour synthesizes, stocks, metabolizes and mostly secretes catecholamines. For this reason it is possible to use elevated concentrations of catecholamines and their metabolic products from plasma and urine as diagnostic markers of this tumour. Determination of metanephrines is often preferred against determination of catecholamines regarding the diagnosis of PHEO. Tumour cells produce free metanephrines continuously and irrespective of the release of catecholamines. The project aims to develop a new kit for the determination of plasma metanephrines. Solid phase extraction (SPE) was used for the pre-treatment of plasma samples. The determination was performed by high performance liquid chromatography with electrochemical detection (HPLC-ED).

Free or total metanephrines for diagnosis of pheochromocytoma: what is the difference?

Pheochromocytomas, although a rare cause of hypertension, are dangerous tumors that require consideration in large numbers of patients. The resulting low prevalence of these tumors among the tested population and the inadequate sensitivity and specificity of commonly used biochemical tests make diagnosis of pheochromocytoma difficult and time-consuming. As outlined in two articles in this issue...

Evaluation of an enzyme immunoassay for plasma-free metanephrines in the diagnosis of catecholamine-secreting tumors.

OBJECTIVE To define the test characteristics of an enzyme immunoassay (EIA) for plasma-free metanephrines (metanephrine and normetanephrine) in the diagnosis of pheochromocytoma and paraganglioma. DESIGN Prospective observational design from a single University Hospital. Twenty-four hour urine for catecholamines and plasma for free metanephrines were collected from patients with a clinical su...

Plasma metanephrines: a novel and cost-effective test for pheochromocytoma.

Pheochromocytomas are rare chromaffin cell tumors that nevertheless must be excluded in large numbers of patients who develop sustained or episodic hypertension as well as in many others with suggestive symptoms or with a familial history of pheochromocytoma. Diagnosis of pheochromocytoma depends importantly on biochemical evidence of excess catecholamine production by a tumor. Imperfect sensit...

Biochemical diagnosis of pheochromocytoma: which test is best?

CONTEXT Diagnosis of pheochromocytoma depends on biochemical evidence of catecholamine production by the tumor. However, the best test to establish the diagnosis has not been determined. OBJECTIVE To determine the biochemical test or combination of tests that provides the best method for diagnosis of pheochromocytoma. DESIGN, SETTING, AND PARTICIPANTS Multicenter cohort study of patients te...

Is supine rest necessary before blood sampling for plasma metanephrines?

BACKGROUND The impact of blood sampling in sitting vs supine positions on measurements of plasma metanephrines for diagnosis of pheochromocytoma is unknown. METHODS We compared plasma concentrations of free metanephrines in samples from patients with primary hypertension obtained after supine rest with those obtained in the sitting position without preceding rest. We also assessed the effects...
